The two-car collision happened on the n25 at Gaulstown in Glenmore at around 6:30pm yesterday.
A woman in her 40s was pronounced dead at the scene while three women, two in their 20s and one in her late teens, have been taken to University Hospital Waterford with serious injuries
The road is closed to allow a forensic examination of the scene take place and local diversions are in place.
Anyone who witnessed the crash is asked to contact Gardai.
