The HSE spent nearly 1.3 million euro on debt-collection costs in 2019 and 2020.
According to freedom of information documents, the money was paid to six companies - with one agency getting over half a million.
The HSE was seeking to collect unpaid costs, such as 100 euro for a patient attending an emergency department.
The Irish Patients Association, is questioning the debt-collection spend.
